APPLICATION FOR PARACHUTE JUMPING EXHIBITION

1. Application is hereby made for a waiver of Section 424, Title 5, VSA, to conduct a Parachute Jumping Exhibition.

 2. The following information is submitted:

     a.  Name(s) and address(es) of participants, with Class of USPA license held.

    b.  Location of drop zone.

    c.  Time and date of jump(s).

    d.  Maximum altitude (MSL) parachutists will exit aircraft.

     e.  Make, model, and "N" number of jump aircraft.

f.  Name, address, and qualifications (type of pilot certificate and experience) of pilot of jump aircraft.

 3.  Applicant will also submit with this application a letter of approval signed by the owner on whose property the jump(s) will be made.

 4.  The provisions of FAR Part 105 will be observed as they relate to this exhibition.
